Medicare Special Needs Plan (SNP)
A Medicare special needs plan is a form of Medicare Advantage Plan that assists people that have special health care requirements. Membership is limited to people with specific diseases or characteristics. To meet the needs of the specific groups they serve, SNPs adjust their benefits, provider choices and drug formularies. You usually need to get your health care services from physicians or health care facilities in the Medicare SNP network. Learn more about SNPs at medicare.gov.
