Advance Directive

Advance DirectiveAdvance directives are legal documents that you create to let your family, friends, doctors and other health care professionals know what kind of health care you would want or who you want to make decisions for you if you’re too ill to speak. Two examples of advance directives are a health care proxy (durable power of attorney) and a living will. You can get an advance directive from your health care provider, attorney, local area agency on aging, or state health department.

George used an advance directive to name his daughter as the one to make health decisions for him if there came a time when he couldn’t himself.
